Remission & chronicity

Spontaneous remission
Progressive with worsening glycaemic control; reversal essentially not possible with current treatments once established; spontaneous improvement only in cases of rapid glycaemic normalisation early in course
Best-intervention remission
Intensive glycaemic control in T1DM (DCCT): 60% reduction in neuropathy development; pain management (duloxetine, pregabalin): 30-50% pain reduction but no disease modification; alpha-lipoic acid IV: nerve conduction velocity improvement at 3 weeks (SYDNEY 2 trial); SGLT2 inhibitors: emerging data on neuropathy protection
Gap size
Very large — no disease-modifying therapy approved; pain treatment is the only intervention most patients receive
Primary barrier
biological (pain modification) + practice (prevention gap)
Prevention through glycaemic control is highly effective but prevention means tight glycaemic control for decades; pain treatment gives symptomatic relief only; opioid prescribing for DPN pain remains common despite guidelines against it; few patients are offered alpha-lipoic acid which has the strongest disease-modification signal

Clinical Trials

Recruiting and recently completed trials from ClinicalTrials.gov. Data retrieved 2026-07-06.

No recent clinical trials found on ClinicalTrials.gov for this condition.

Agents Found By Disease-Level Search

Searched directly from “Peripheral Neuropathy (Diabetic Peripheral Neuropathy)” via ClinicalTrials.gov interventions and Open Targets' disease→drug data — independent of the 0-gene target panel above. This surfaces agents whose mechanism doesn't route through a curated gene (combination therapies, standard-of-care drugs, targets outside the panel).
